In a O'Brien B214TH-MK505-Ts3115D2CJS1H-W1C enclosure.

At the time of this listing we have a quantity of these but they are all new and identical to the one shown in the pictures. They were dusty and when I cleaned them off with a spray on cleaner I found that the "ON/OFF stickers on the switches on the side are made of paper and not plastic like the other stickers and the ink on those 2 stickers wiped right off (see last 3 pictures).
